HOW TO REGISTER FOR THE CONFERENCE

After reading the instructions below, click on the appropriate
icon at the bottom of the page to identify your registration
status and begin the registration process. Please read all
of the instructions below carefully!

Providing Contact Information (Step
1 of 2)
|
|
Click on the appropriate icon at the bottom of the page to
send your contact information to Health Systems Research,
Inc., the logistical support contractor for this conference.
Required fields are indicated with red asterisks (*).
Please check to make sure that your e-mail address is entered
correctly. When the form is complete, click on the Submit
button.
Shortly after submitting your registration information, you
will receive an automated e-mail message. This message will
confirm receipt of your registration information and provide
additional contact information and updates.

Secure Electronic Payment (Step
2 of 2)
|
Upon successfully completing the form, you will be linked
over to a secure payment form to process your appropriate
payment amount. Payment must be submitted at the time
of registration or your registration is invalid.
The registration fee is refundable up to June 11th. All
requests for a refund prior to June 11th will receive a
full refund. There will be no refunds on registration fees
after June 11th.
You will receive a separate payment confirmation
e-mail receipt from EMS, the credit card processor, when
the transaction is approved. Currently, EMS accepts VISA,
MasterCard, and American Express.

To Register By Fax or Mail
|

If you are unable to register online for any reason (e.g.,
you do not have a valid e-mail address or do not have a credit
card to process payment), please go to the printer-friendly
registration form. This form should be printed, completed,
and faxed to (202) 296-2663 (a secure fax machine) or mailed
(along with a check, if applicable, for the appropriate amount)
