Providing Contact Information (Step
1 of 2)

Click on the appropriate registration fee at the bottom
of the page to select whether you will be attending both
the pre-conference workshop and the main conference or only
the main conference. You will then be taken to a form to
send your contact information to Health Systems Research,
Inc., the logistical support contractor for this conference.
Required fields are indicated with red asterisks (*).
Please check to make sure that your e-mail address is entered
correctly. When the form is complete, click on the Submit
button.
Shortly after submitting your registration information,
you will receive an automated e-mail message. This message
will confirm receipt of your registration information and
provide additional contact information and updates.

Secure Electronic Payment (Step
2 of 2)

Upon successfully completing the form, you will be linked
over to a secure payment form to process your appropriate
payment amount. Your registration is not valid until
payment is received.
The registration fee is refundable up to Wednesday,
March 9th. All requests for a refund prior to March
9th will
receive a full refund, minus a $45 administrative fee.
There will be no refunds on registration fees
after March
9th although you can
send a replacement from your organization or company. Please
notify us if you send
a replacement to this conference.
You will receive a separate payment confirmation
e-mail receipt from EMS, the credit card processor, when
the transaction is approved. Currently, EMS accepts VISA,
MasterCard, and American Express.

If you are unable to register online for any reason (e.g.,
you do not have a valid e-mail address or do not have a credit
card to process payment), please go to the printer-friendly
registration form. This form should be printed, completed,
and faxed to (202) 828-9678 (a secure fax machine) or mailed
(along with a check, if applicable, for the appropriate amount)

Registration Deadlines & Questions
|
The Early Bird registration deadline is Tuesday,
February 15th,
however space is limited so we urge you to register now.
Please note that after February 15th the registration
fee will increase $75 for PHPC members and non-profit/Government
subscribers to Rx for Access or the Federal Drug Discount
and Compliance Monitor, $125 for non-profits/Government,
and $200 for industry representatives. The
final registration deadline for this conference is Wednesday,
March 2, 2005.

(March 16, 1:30 pm to March 18, 12:15 pm) OR only
the main conference (March 17, 7:00
am to March 18, 12:15 pm). The pre-conference
workshop is a three hour and a 45 minute session
for those interested in learning the basics of the
340B program. Note: You cannot register for the pre-conference
workshop only, you must register for the main conference
as well.
