  • The Health Care Quality Improvement Program (HCQIP) was initiated in 1993 by Island Peer Review Organization (IPRO) under contract with the Health Care Financing Administration (now known as the Centers for Medicare and Medicaid Services) of the U.S. Department of Health and Human Services for the purpose of assuring quality of care for Medicare patients. The purpose of the HCQIP is to develop information on patterns of care and outcomes and to share this with health care providers in order to effect measurable improvement in care and outcomes for Medicare beneficiaries.

Under the three-year Medicare "Sixth Scope of Work," which began on August 1, 1999, IPRO and New York hospitals and managed care plans will collaborate on national and local quality improvement projects.

This session will offer participants an overview of the National Quality Improvement Project that reflects six national clinical priority areas: acute myocardial infarction, breast cancer, diabetes, heart failure, pneumonia, and stroke.

Participants will hear, firsthand, from hospitals who are participating in these important projects and their results to date.

  • Improving Medications Management

The Institute of Medicine (IOM) Report, To Err is Human, acknowledges that ensuring appropriate medication use is a complex process involving: multiple organizations and professionals from various disciplines; knowledge of drugs; timely access to accurate and complete patient information; and a series of interrelated decisions over a period of time. Errors can creep into this process at various points, including errors of commission (e.g., failure to administer a drug that was prescribed).

This session will address innovative approaches to improving patient safety that have been undertaken by hospital pharmacies.
