Please read the instructions below on how to register for the Web-assisted audio conferences. When you are ready, click on the button at the bottom of the page to register for any or all three of the Web-assisted audio conferences.

Please Note:  We are experiencing an extremely high volume of registrations for this series. This may cause some audio connection difficulties for those connecting at, or shortly after, the beginning of the program. If for any reason you are unable to access the audio portion of the program via your computer, you may dial in using your telephone at any time by calling the following phone number: 1-888-868-9080 and use the passcode: "Bioterrorism Audio Conference".

Providing Contact Information

Click on the button at the bottom of the page to complete and submit the online registration form to Health Systems Research, Inc., the logistical support contractor for this conference.

Required fields are indicated with asterisks (*). Please check to make sure that your e-mail address is entered correctly. When the form is complete, click on the Submit button.

A few seconds after submitting your contact information, you will receive a registration confirmation page. This Web page will confirm receipt of your registration information. You will also receive an automated e-mail message providing you with instructions, additional information, and updates, including information on how to access the audio conference from both Web and phone. We recommend that you print this e-mail and save it for your records.

To Register By Fax

If you are unable to register online for any reason (e.g., you do not have a valid e-mail address), please go to the printer-friendly registration form. This form should be printed, completed, and faxed to:

Upon receipt of your fax registration, HSR, Inc. will fax back to you a registration confirmation, providing you with instructions, additional information, and updates, including information on how to access the audio conference(s) from both Web and phone.

Testing Your Computer

To test your internet connection and pc compatibility prior to registering for an event, please follow the following steps:

• Go to the Communicast Web site at: http://communicast.com.
• Click on the "Participant Login" button at the top of the screen.
• Instead of logging in, click on the "Test My Computer First" button at the bottom of the screen, which will take you through a series of steps to test your system.
