How to Register

Click on the button at the bottom of this page to complete the online registration form and send your contact information to Health Systems Research, Inc., the logistical support contractor for this conference.

Required fields are indicated with asterisks (*). Please check to make sure that your e-mail address is entered correctly. When the form is complete, click on the "Submit" button.

Shortly after submitting your registration information, you will receive a registration confirmation e-mail. This e-mail will confirm receipt of your registration information and provide additional information and updates. We recommend you print this out and keep it for your records.

To Register by Fax or Mail

If you are unable to register online for any reason (e.g., you do not have a valid e-mail address), please go to the printer-friendly registration form. This form should be printed, completed, and faxed to (202) 728-9469 or mailed to:
