The workshops are open to representatives of public and private organizations interested in applying for grant funding under this program. There is no registration fee to attend a workshop, and registration is limited to one person per organization. You must register in advance to attend a workshop, and space is limited. Consequently, please register early to ensure a spot in a workshop.

Providing Contact Information

Select the workshop location you wish to register for at the bottom of this page to complete the online registration form and send your contact information to Health Systems Research, Inc., the logistical support contractor for this conference.

Required fields are indicated with red asterisks (*). We ask that you please do not use any acronyms when filling out the registration form. Please check to make sure that your e-mail address is entered correctly. When the form is complete, click on the Submit button.

Shortly after submitting your registration information, you will receive a registration confirmation e-mail. This e-mail will confirm receipt of your registration information and provide additional information and updates. We recommend you print this out and keep it for your records.
