The deadline to register for the meeting is August 31. The deadline to request an opportunity to provide oral testimony is August 22.

Click on the button at the bottom of the page to complete the online registration form and send your contact information to Health Systems Research, Inc., the logistical support contractor for this conference.

Required fields are indicated with asterisks (*). Please check to make sure that your e-mail address is entered correctly. When the form is complete, click on the Submit button.

Shortly after submitting your contact information, you will receive a registration confirmation page. This Web page will confirm receipt of your registration information. You will also receive an automated e-mail message providing you with instructions, additional information, and updates. We recommend that you print this e-mail and save it for your records.
