final report includes a proposal for the State of Texas to improve standardization
across case management programs and recommendations for ensuring that
individuals being served through both primary care case management and
risk-based managed care arrangements receive levels of case management

implementing risk assessment systems under Medicaid managed care for
three populations of critical concern: pregnant women, children with

For the
State of South Carolina, HSR facilitated the initial integration of
Maternal and Child Health (MCH) services within the South Carolina Department
of Health and Environmental Control (DHEC) and Medicaid managed care.
The report presents a draft contract between Medicaid and MCH defining
a package of "wrap around" Family Support Services-including
outreach, risk assessment, case management, health education, nutritional
services, social work services, and caregiver training-that could continue
to be provided by DHEC providers and reimbursed by Medicaid within a
